This is a Home and Community Based Services ( HCBS ) program administered by the New York State Department of Health ( DOH ) to assist individuals with disabilities and seniors toward successful inclusion in the community, especially participants living in the Nursing Home. Since no two clients are the same, Jarme provide a customized plan of care adjusted to your needs and Health condition. we work greatly with patients family and Physicians to ensure that their recovery at Home is achieved with dignity.
