SeniorHabitat was created because 87% of caregivers are unaware of resources that can help them make critical decisions and provide proper care. Although the United States is one of the most advanced country when it comes to medicine, it is still struggling with providing high-quality care to older adults. Rather than providing high-quality care, services is often delivered by many different providers without coordination. As a result, many older adults aren't getting their needs met and caregivers are struggling with their growing responsibilities. Rather than having caregivers feel ‘left in the lurch’ and having to “battle the system,” SeniorHabitat empowers care connections by using technology to improve continuity of care and provide better access to support services. It serves the individual needs of older adults through interactive, intelligent-driven care solutions. To improve quality of life for older adults, SeniorHabitat created a platform that provides support and guidance to stressed family caregivers by offering objective and qualified solutions for the following social determinants of health: 1) Resources to meet daily needs (ex. housing & nutrition), 2) Access to health care services, 3) Community-based resources (ex. care management), 4) Social & legal support, 5) Opportunities for recreational & leisure activities (ex. adult day & senior centers), 6) Transportation options. Our mission is to improve the quality of life (QoL) for older adults by enabling access to a comprehensive network of care providers and services.
